If you have a public event that you would like to request Navy band support please follow these steps:

Step One

Open the current DoD 2536.pdf.

Step Two

If you have Adobe Acrobat Standard or Professional, you may fill in the form electronically then print off a hard copy

OR

Print blank form first and fill out by hand (** Ink signatures required in box 23a. **)

Identify the Navy Band that serves your area and scroll down for contact information and more about the band. (many have their web sites)

Fax the completed form to the band. It is recommended that you phone first and make contact with the band’s Operations Coordinator to discuss the band’s availability and explain the process further. If you have fax difficulties or want to confirm which band serves your area please contact the NAVCO Band Support Officer at 901-874-5804.
