Performance Evaluation Error Codes Are Not Board Disqualifiers

MILLINGTON, Tenn. (NNS) -- Navy Personnel Command (NPC) is reporting a slight increase in rejected evaluations due to a change to the Navy's physical fitness assessment (PFA) codes, which was implemented Aug. 1 in NAVADMIN 193/10, said Navy Personnel Command's (NPC) Performance Evaluation Division.

In order to ensure the upcoming boards have a complete record to review, affected Sailors can submit the uncorrected fitness reports and evaluations as official correspondence directly to the selection board for consideration.

For the purpose of the upcoming boards, NPC will accept uncorrected fitness reports and evaluations as an interim measure to ensure Sailors have performance evaluation continuity and are not penalized for a command's submission of evaluations containing error codes. Of the more than 6,200 evaluations rejected during August and September 2010, only 4,500 or about 8 percent of the total evaluations submitted were rejected because of incorrect PFA codes.

"The overall reject number is not much different than the historical averages," said Jim Price, Performance Evaluation Division director. "However, over the past 12-15 months we had been averaging around a four percent overall reject rate. So when we saw the spike in August and September, we did further analysis and found that the increase was directly related to the PFA code change."

While this interim fix will ensure Sailors have complete records, these submissions will only apply to the board and will not be entered into their official military personnel record. Commands will still be required to submit a corrected version to the Performance Evaluation Division.

Although a system is in place to account for these records, "commands still need to fix the reports," said Price.

To fix the reports, the original reporting senior can:
• Retype the original report.
• Make pen and ink changes on a copy of the report and initial the changes.
• White-out or tape over the error and initial the changes.

"If the correction changes the competitive grouping, corrections must be made to all the reports in the summary group," explained Price.

Reporting seniors must ensure a copy of the corrected report is provided to the Sailor or forwarded to their new command. If a report was rejected due to a missing member signature, and the report is not adverse, the reporting senior can write "certified copy provided" in the member signature block.
