Connect With Health Care Anytime, Anywhere - With RelayHealth Patient Portal

Use it to email the health care team, request non-urgent appointments, get lab results, renew medications, access a health library, and get health e-news.

"It saves time for routine questions like lab tests. And signing up was easy," said retired chief petty officer Dave Bragg, a 3-year RelayHealth user. "If I can do it, anyone can."

RelayHealth is secure and offers 24/7 accessibility. Patients generally get a response from their health care team within one business day.

"Our RelayHealth patient portal connects you to your health care team from any mobile device or computer - whenever and wherever you need," said Capt. Troy Borema, NH Jacksonville director for medical services and family medicine physician. "It can save a phone call or sometimes even a trip to the doctor."

RelayHealth is one of the advantages of being enrolled with a PCM and Medical Home Port Team at NH Jacksonville - its hospital or branch health clinics. The team focuses on meeting the patient's needs, for urgent, preventive and routine care. Each team has also earned highest-level recognition from the National Committee for Quality Assurance.

NH Jacksonville's priority since its founding in 1941 is to heal the nation's heroes and their families.
