At a minimum, each CLJA claim must include the following documents so we can determine if you meet the eligibility requirements. Sign into your Claims Management Portal account to upload these documents. 1. Photo Identification. Provide a copy of your most recent government issued identification. 2. Proof of Exposure. Provide proof you lived or worked at Camp Lejeune for at least 30 days between August 1, 1953 and December 31, 1987. Acceptable proof includes but is not limited to: Military records (DD-214), employment records, school records, court records, letters addressed to the claimant reflecting the claimant's address, dated photographs showing physical presence at Camp Lejeune, etc. If the only proof you have is a DD-214, it must indicate presence at Camp Lejeune between Aug. 1, 1953 and December 31, 1987. 3. Evidence of qualifying injury. Provide signed and certified medical records which include the date of your diagnosis and/or treatment for the injury that you claim. If you are an attorney filing on behalf of a claimant or a claimant's authorized agent, please provide the following: 1. Fully executed attorney authorization 2. Claimant or claimant's authorized agent's photo identification 3. Proof of exposure 4. Evidence of qualifying injury If the claimant is deceased, subject to a guardianship, or an attorney-in-fact is acting on behalf of the claimant, you must also provide one of the following: 1. Deceased: Please provide a PDF copy of the Letters of Administration or Letters Testamentary which bear a clearly visible seal of the court. 2. Guardianship: Please provide a PDF copy of the Letters of Guardianship which bear a clearly visible seal of the court. (Note: The scope of the guardianship must include the power to pursue legal claims.) 3. Attorney-in-Fact: Please provide a PDF copy of the current, valid, Durable Power of Attorney and a notarized Certificate of Power of Attorney. (Note: The Power of Attorney must include the power to pursue legal claims.) Submit supporting documents within the online claim portal. The Department of the Navy will notify you of your options, in writing, if your claim is denied.
